Follow-Up: I, too have bruxism and TMJ. I also suffer from sleep apnea due to a receding lower jaw. I've been on CPAP and at one time I used a lower jaw appliance, but the most effective treatment I've found for all three disorders consists of an appliance that was contructed by the dentist who was treating my TMJ. The appliance consists of a mouth guard made out of acrylic. It has clamps on the upper and lower portion of the device so that when I bite down on it my upper and lower teeth are held in place. It's fitted in such a manner so that my jaw extends slightly outward and firmly in place while I'm asleep. This increases the size of my breating passage (apnea), realigns my jaw (TMJ), and keeps me from grinding my teeth. When I was on CPAP, using a nasal mask, headband (to keep my mouth shut) and TMJ appliance, I was able to get a pretty good night's sleep, usually about 6 to 7 hours. Prior to CPAP, I could only average about 3 to 4 hours of sleep each night, with constant grinding and nocturnal panic attacks. With this new appliance, I sleep as well as I did with CPAP, without all of the equipment attached to my head. If you would like additional information this, please feel free to email me. I just happened on to this site and I don't know if I'll get back to it. I could also take a picture of the device and send that to you via email attachment, along with the name of the dentist who made it for me.
